Cancer diagnoses are among the most frequently unsupported codes in Medicare Advantage audits. Many errors stem from coding active cancer when the documentation reflects history, remission, or no current treatment or evaluation.
Key documentation points for cancer diagnoses:
Specify cancer status: active, in remission, history, or metastatic.
Show current clinical relevance: monitoring, oncology coordination, treatment decisions, or symptom evaluation.
Avoid relying on the problem list without confirming status during the encounter.
Use history codes (Z85‑) when treatment has ended and no active disease is present.
Document metastatic sites clearly when applicable, avoiding vague terms like “spread” or “advanced.”
These steps help ensure that cancer diagnoses are accurate, defensible, and aligned with OIG expectations.
